Abstract
Clinical pharmacists make a huge contribution towards increasing medication safety and optimizing drug therapy in hospital environments. This study aims to evaluate the role played by clinical pharmacists at medication safety, reduction of medication errors and adverse drug reactions (ADRs), enhancement of medication therapy management, and collaborative efforts in interdisciplinary healthcare teams. The significance of clinical pharmacists in developing patient-centred care has been established by analysis of various interventions and case studies in hospitals. Findings suggest that even membership alone leads to substantial reductions in medication error, thereby improving patient adherence and boosting delivery of health care. Detailed statistical and case analyses in different hospitals make a point about integrating clinical pharmacists into patient care teams. Clinical Pharmacy as a field is growing to immense heights as the healthcare on the global level requires new advancements in terms of technology, knowledge, methodologies and quality patient care. The role of clinical pharmacist is a broader aspect which by the means of this study is depicted to understand their responsibilities, duties and dynamic aspects of the profession to enhance patient care and provide safe and effective medication use in hospitals.
